As a healthcare worker, you spend a great deal of time taking care of others. Now it is time to take good care of yourself. Because we are committed to the health and well-being of our employees and their families, Saint Francis Medical Center offers Healthy Rewards, a FREE program designed to help you improve or maintain your health.

All Saint Francis part-time and full-time employees can participate in Healthy Rewards. Spouses are also encouraged to participate if enrolled in the Medical Center's health insurance plan. While Healthy Rewards is FREE, the value to you can be more than $1,500 per year, including up to $350 earned in vouchers , not including the monthly discount1 you can receive on your health insurance premium. 1 Spousal participation is required.

Goal Settingtop

Healthy Rewards is based on eight wellness categories:

  1. Blood pressure
  2. Blood sugar
  3. Body weight
  4. Cholesterol
  5. Exercise
  6. Safety and prevention
  7. Tobacco use
  8. Wellness

Wellness staff will help you set realistic goals for each category. Each category is worth 100 points, and you can earn points throughout the year by meeting your individual goals. There are also optional programs each month to help you earn additional points. These are listed in the Program Calendar.

Annual Enrollment Instructions. Enrolling Is as Easy as 1, 2, 3 – Just Follow the Simple Instructions.top

Enrollment or annual reassessment must be completed in the month before, during or after your annual evaluation.

STEP 1: Complete a Personal Wellness Profile™ (PWP). This must be done before completing Step 2. Failure to complete your PWP before having labs drawn will result in the loss of Healthy Rewards vouchers.

Follow the directions below to complete your PWP online:

  • Access your Healthy Rewards account at https://wellsuite.com/sfmchealthierus/ws/ from home or work. If you have not already activated your online Healthy Rewards account, please refer to the instructions on the on the Annual Enrollment Instructions PDF (located after STEP 3). Click the Personal Wellness Profile™ tab under Healthy Rewards tools to bring up the page.
  • Click “Start an Assessment.”
  • Click “Agree” on the terms and conditions to begin the questionnaire (if applicable).
  • Fill out all questions to the best of your ability for all 13 sections of the questionnaire. Section 12: Clinical Data and Section 13: Additional Questions will be blank.
  • When satisfied with your answers, click “Finish” to save your assessment.
  • When saving your assessment, you will be given the option to title the assessment you just completed. It is recommended that you use the default title of date and time that is already entered for you.
  • Once you save your assessment, you will have 24 hours to change any answers.

STEP 2: Have your lab work drawn. There are two ways for you (and your spouse, if applicable) to complete your lab work. You must have your PWP completed online before your labs are drawn. Employees and spouse labs do NOT have to be completed on the same date.

  • Express Screening2 – preferred method (faster for you, faster for Wellness):
    Attend the Express Screening the month before or during your evaluation month. Express Screenings are typically held the first Thursday of each month from 7 to 9 am in the Health and Wellness Center Conference Rooms. The following are required for the Express Screening:
    • Fast 10-12 hours (water and medication are allowed)
    • Complete online PWP assessment

  • Individual Lab Draw3 – alternative method:
    Pick up a lab slip at Employee Health or the Fitness Plus Service Desk. You (and your spouse, if applicable) will have to sign stating you already have completed your online PWP assessment. Call Scheduling at (573) 331-5111 or ext. 6211 to set up appointments for your lab work and your Healthy Rewards annual appointment. Spouses can have separate appointments.

STEP 3: Attend your annual Wellness appointment. You will receive your lab work, PWP report and any vouchers you have earned. If you are not currently receiving a discount on your health insurance, it will begin no later than 30 days after completing Step 3. Only employees are eligible to earn vouchers.

2 Dependents who want to take advantage of the free lab work must have their labs drawn during an Express Screening. 

3 Method B is not available to dependents.

How to Activate Your Healthy Rewards Account:
  1. Visit https://wellsuite.com/sfmchealthierus/ws/
  2. Enter your username and default password. Employees will enter their employee number; spouses will enter a username, see below.
    Username: first initial, middle initial, last name (e.g. John A. Smith = jasmith); Password:sfmc)
  3. You will be prompted to reset your password. Your new password may consist of letters and/or numbers but must be at least four characters.
  4. Please fill in all the contact information, especially the email address.
  5. Select a password reset question in case you forget your password.
  6. Enter the required health stat estimates (age, height, weight, waist girth). Wellness will do an actual assessment at an Express Screening or during your annual evaluation.

Enrollment for Dependents Age 18 and Older:

Employees’ dependents 18 years and older who are enrolled in the Medical Center’s health insurance plan are eligible to participate in the Healthy Rewards Express Screenings the month before, during or after the employee’s annual evaluation month. Participation by dependents is a bonus benefit and does not positively or negatively affect the insurance premium reduction or incentives earned by the employee.

Dependents who want to participate must complete the following before attending the Express Screening:

  1. Contact the Wellness department at 573-331-5970 or email healthyrewards@sfmc.net and leave your full legal name and phone number so your online PWP can be activated. Please allow two business days for this to be completed.
  2. Complete the online PWP using the following username and password:
    Username: (first initial, middle initial, last name)
    Password: sfmc (lower case)
  3. Fast 10-12 hours (water and medication are allowed) before attending the an Express Screening

Rewardstop

To be eligible for rewards, you must participate in your annual re-evaluation and/or other quarterly or midyear checks, which may be required and are determined at your initial consultation with Wellness. Each year during your annual re-evaluation, all participants – regardless of how many goals are met or points are accumulated – receive $50 in vouchers for completing the PWP and having the annual lab work done. Other incentives, such as water bottles and healthy cookbooks, are given out during programs as well.

Participants can earn money vouchers – up to $350  – for meeting their goals. Vouchers can be used toward Saint Francis insurance premiums or medical bills, Fitness Plus membership fees and services, Wellness services and items from Pharmacy Plus or the Healing Arts Pharmacy. Voucher amounts are based on the total points earned during the year and the number of consecutive years of participation in Healthy Rewards (see chart below) and are distributed within one month of your annual re-evaluation.

What is Healthy Rewards?

Healthy Rewards is a FREE Wellness incentive program for full-and part-time staff designed to help employees improve their health or maintain their good health. It is a positive reinforcement program in which healthy behaviors are rewarded. The program is tailored to individual needs. Healthy Rewards goals are based on eight different categories: wellness activities, safety and prevention, tobacco use, exercise, body weight, blood pressure, cholesterol, and blood sugar.

When can I join Healthy Rewards?

During the month in which you are hired OR at the time of your annual evaluation with your supervisor.

How much does it cost?

Absolutely nothing! You never have to pay a dime for Healthy Rewards; you can only EARN money.

Do I have to be a member of Fitness Plus to be in Healthy Rewards?

No, you do not have to be a member of Fitness Plus.

How does the Healthy Rewards points system work?

Participants can earn up to 100 points in each category throughout the year, for a total of 800 points. Money vouchers – up to $350 – are awarded at the end of each year of participation by the total amount of points earned and the number of consecutive years of participation (see chart below). Vouchers are distributed within one month after re-evaluation with Wellness. To be eligible, you must participate in the re-evaluation and/or other quarterly or midyear checks, which are determined by your initial screening results.

Do I have to exercise?

No, but you can earn points for exercising and keeping documentation of it.

Do I have to attend various Wellness programs?

No, but you can earn points for attending programs, as well as completing quizzes on the NetLearning system and participating in Wellness Challenges.

Do I get a discount on my insurance premium?

Yes. When you enroll for the first time, you will begin receiving a discount on your monthly insurance premium and will continue to get this discount throughout your first year in the program. To keep the discount, you will need to re-enroll in Healthy Rewards each year during the time of your annual evaluation, and you must have earned at least 400* out of 800 possible points.

Tobacco Cessation Information

This six-week program offers coping strategies, encouragement, support and techniques to empower smokers to quit. Financial assistance for nicotine therapies is available. Call ext. 3006 or 573-275-2177 with questions. Registration is open only during the first class. Cost is $70 for employees for the six-week program and is due at the first class or may be deducted from two pay periods. Six-week program starts on January 9, March 6, May 1, June 19 and August 21 in Health and Wellness Center Conference Room 3, meeting every Wednesday at 7 am and 7 pm. The sessions last 30-40 minutes.
