Home Health
The Home Health program at Saint Francis Medical Center features caring, highly skilled therapists, nurses and aides who provide various healthcare services to people confined to their homes. Individualized plans of care are developed for qualified patients based on input from physicians, Home Health staff and at-home caregivers. Services provided by Home Health include:
Physical therapy services include walking, gait training, home exercises, prosthetic instructions, rehabilitation and restorative therapy.
Occupational therapy services include assessing patient needs and providing assistance/instructions to improve daily living activities through alternative means and assistive devices.
Speech therapy services include evaluating, assisting and instructing patients and their at-home caregivers on how to improve speech, cognitive and swallowing problems.
Medical social services include assessing and counseling those with social and emotional needs and coordinating community resources assistance.
Home health aides services include assisting with daily living activities and providing personal care (available only when skilled nursing or therapist is ordered).
Skilled nursing services include observation, assessment, postoperative follow-up treatment, medication evaluation, laboratory sample collection, dressing changes, catheter/ostomy care, injections, administration of IVs, patient/caregiver teaching and physician reports. An on-call registered nurse is available 24 hours a day, seven days a week.
A wide variety of home infusion services are available, including IV antibiotics, IV/SQ pain management, TPN (total parenteral nutrition) and IV hydration therapy administered by a qualified registered nurse who also provides patient training and monitoring. An on-call registered nurse is available 24 hours per day, seven days a week.
