Each holiday season, Saint Francis colleagues come together to support families in need through our Adopt-A-Family Christmas Program. This year, we are focusing on supporting our own colleagues. Departments and clinics across the Healthcare System will “adopt” colleagues in need of assistance and supply them and their family members with gifts for the holidays. To apply for the Adopt-A-Family program, please fill out the form below.
Application FAQs
- Colleagues who apply for the Adopt-A-Family program must be employed for a minimum of 6 months.
- Colleagues must fill out the form in its entirety to be considered.
- Colleagues must fill out the application form themselves and may not have another colleague fill out the form on their behalf.
- Colleagues will receive a confirmation e-mail once their application has been submitted. Colleagues who do not receive a confirmation e-mail should contact Emily Vines by e-mail at evines@sfmc.net.
- Colleagues will be notified of their application status within two weeks of submitting their application.
- Items can be picked up at Saint Francis Medical Center.
