Healthy Rewards is a voluntary program designed to reward colleagues and spouses for taking an active role in their health. See below for the opportunities to choose your level of participation.
What is Changing for 2027?
While the way participants earn incentives is changing, our commitment to supporting the health and well-being of Saint Francis colleagues remains the same.
Beginning in 2027, Healthy Rewards will no longer offer monetary premium reimbursement incentives due to changes in our medical plans. Instead, participants can earn entries into quarterly prize drawings by completing wellness activities throughout the year.
Even better, Healthy Rewards is expanding to include all Saint Francis colleagues, including PRN colleagues, those not enrolled in a Saint Francis medical plan and eligible covered spouses.

How Can I Earn Rewards?
Healthy Rewards focuses on three areas: Know Your Health, Stay Engaged and Get Support.
Participants can earn prize drawing entries by completing activities such as:
- Annual preventive exams
- Health Risk Assessments
- Exercise goals and wellness challenges
- Wellness programs and classes
- Employee Assistance Program (EAP) activities
- Monthly wellness articles
- Health coaching sessions
The more activities you complete, the more entries you earn!
Each quarter, prizes will be awarded, including athletic shoes, smartwatches, earbuds, fitness training sessions, outdoor recreation items, reserved parking and more.
Health Coaching Is Available at No Cost
Healthy Rewards also offers free, confidential and voluntary health coaching to help participants reach their wellness goals. Whether you are interested in improving nutrition, increasing physical activity, managing stress, improving sleep or preventing chronic conditions, our health coaches can provide personalized support.
Plus, participating in health coaching earns additional prize drawing entries!

Are you already participating in Healthy Rewards? Keep going! Continue submitting your annual preventive exam documentation, logging in to ManageWell and completing wellness activities to earn prize drawing entries throughout the year.

- Explore available activities and choose a wellness goal that works for you.
You do not have to do everything to benefit from Healthy Rewards. Every step toward better health counts!

ManageWell
Set up your ManageWell account to access health and wellness resources and manage Healthy Rewards.
Savings
Better health is rewarding. For each level of Healthy Rewards that you participate in, the savings stack higher and higher.
Eligibility
All colleagues and spouses covered by the Saint Francis Healthcare System medical insurance plan are eligible to participate.
Level 1
Complete after Jan 6
Sign in at managewell.com and agree to the website / app terms after January 6 to receive the Level 1 $5 monthly premium reimbursement.
Level 2
Due by Feb 28 (to be eligible for Level 3)
Complete your annual preventive exam and labs with a provider. Have the provider submit a Healthy Rewards Ambulatory Referral to receive the Level 2 $5 monthly premium reimbursement.
Level 3
Level 1 & 2 must be completed to be eligible for Level 3
Participate in monthly activities to be eligible to earn the Level 3 10-20% monthly premium reimbursement.
Returning Healthy Rewards Participant: Sign in at www.managewell.com.
First-Time Healthy Rewards Participant:
- Visit www.managewell.com from your computer web browser. You can also download the free App (ManageWell 2.0) and follow the prompts to complete your online registration from your smart phone.
- Enter the email you wish to use for your ManageWell login.
- If you are an employee, enter the initials of Saint Francis Medical Center (SFMC) followed by your employee number (Example: SFMC12345). If you are a spouse, enter the primary covered colleague’s employee number followed by the letters SO (Example: SFMC12345SO).
- Enter YOUR date of birth, as this will be tied to YOUR personal account.
- Read through the Healthy Rewards Participant’s Rights, Responsibilities and Program Disclosures. Once you acknowledge the disclosure it will take you to the home page of ManageWell.
Once you are logged in to your ManageWell account, select “Rewards / Messages / Goals.” You will see a golden chalice if you have completed the Level 1 and / or Level 2 requirements. You must complete Level 1 & 2 enrollment to be eligible to participate in Level 3 each program year.
Health coaching is available to all Level 3 participants free of cost. Those that are on a Chronic track in Healthy Rewards are assigned a Health Coach. This telephonic based program will help you understand how lifestyle changes can help you reach your health goals. Trained coaches will provide you with the tools and support you need to make changes in your lifestyle that will help you reduce risks and improve your health. When you work with a coach, together you will create an action plan and set realistic goals to help you make behavior changes. To find the Health Coach assigned to you, visit your ManageWell portal.
Your monthly reimbursement will also be referred to as your “monthly reward”. To earn your Level 3 reward each month, you must complete the activities in your ManageWell account each month. Click on “Learn More” next to the current month to see the activities for the current month. As activities are completed a green checkmark will appear. Once you have completed all required activities, the chalice for the month with turn golden.
This is determined by the eligible members on your SFHS insurance plan.
- If it just you on your insurance plan, then you will earn a 20% premium reimbursement each month that you complete all Level 3 requirements.
- If a spouse is on your insurance plan, he/she is also eligible to participate in Healthy Rewards. You EACH can earn a 10% premium reimbursement each month that you each complete all Level 3 requirements.
The dollar amount that the 10-20% reimbursement equates to on your paycheck depends on your personal medical insurance plan.
Healthy Rewards is committed to helping you achieve your best health. If you believe that an existing or newly developed health issue may make it unreasonably difficult or medically inadvisable for you to attempt achievement of a goal established in Healthy Rewards, you are encouraged to request a Reasonable Alternative Form. Your primary care provider must complete the Medical Condition Statement summarizing your health condition so that a reasonable alternate behavioral health goal may be developed for you.
