Healthy Rewards is a voluntary program designed to reward colleagues and spouses for taking an active role in their health. All colleagues and spouses covered on the Saint Francis Healthcare System medical insurance plan are eligible to participate. See below for the opportunities to choose your level of participation.

What’s New for 2024?

  1. Bigger premium reimbursements available to ALL participants!  You choose your level of participation.  The more you do the more you can earn.
  2. Earn $5 monthly premium reimbursement all year long just for activating your Healthy Rewards Account – no strings attached!
  3. Fitness/gym visits will be an additional option for tracking physical activity.
  4. Those with multiple chronic conditions will be able to select one for improvement instead of all.
  5. Those on the chronic track will have more program activity options available to choose from.
  6. The deadline to get the annual preventive exam has been extended by three months (February 28).

Complete Healthy Rewards Online Enrollment

During Medical Benefit Enrollment (October 22-November 5) you will be asked if you would like to participate in Healthy Rewards. You must mark YES. After benefit enrollment closes, you will be sent an email with directions on how to activate your ManageWell account. Set up your ManageWell account for access to health and wellness resources.

Nothing else will be required of you to receive this reimbursement through December 2024.
Complete a Preventive Exam / Labs and Healthy Rewards referral.

Schedule an annual preventive exam and labs with your provider and have him / her submit a Healthy Rewards Ambulatory Referral.

You will receive a confirmation email from Healthy Rewards on receipt of your referral. Nothing else will be required to receive this reimbursement through December 2024.

*Referral must be submitted by February 28, 2024 to be eligible for Level 3.
Monthly Program Participation

Complete monthly activities to be eligible to earn an additional 10-20 percent reimbursement through December 2024.

Eligible for additional 20 percent – Colleague
Eligible for up to additional 20 percent – Colleague (10 percent) + Spouse (10 percent)
