Saint Francis Clinics Achieve Patient-Centered Medical Home Recognition

Five Saint Francis Healthcare System primary care clinics have achieved recognition by the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home Program: Saint Francis Clinic Farmington, Saint Francis Clinic Poplar Bluff, Saint Francis Clinic Jackson, Saint Francis Clinic Kingshighway and Cape Physician Associates. These five clinics join five other Saint Francis primary care clinics – located in Sikeston, Charleston, East Prairie, Scott City and Dexter – with this recognition.

The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement. It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts.

It fosters collaboration, prioritizes patient well-being, improves quality, emphasizes team-based care and contributes to a more efficient and effective Healthcare System,” stated Ginger McCord, MSN, RN, CHSP, Director of Care Management for Saint Francis Healthcare System.

NCQA is a private, nonprofit organization dedicated to improving health care quality. The Recognition Programs assess whether clinicians and practices support the delivery of high-quality care and are built on evidence-based, nationally recognized clinical standards of care.

O’Kane. “Recognition shows that these Saint Francis clinics have the tools, systems and resources to provide its patients with the right care, at the right time.”
