Healthy Rewards is a voluntary program designed to reward colleagues and spouses for taking an active role in their health. The program has been completely revamped from what you might have known before. All colleagues and spouses covered on the Saint Francis Healthcare System medical insurance plan are eligible to participate.
Colleagues on the Saint Francis medical insurance plan as individuals or with children only (no spouse) can earn a 20 percent premium reduction. Colleagues with a spouse on the Saint Francis medical insurance plan can earn a 10 percent premium reduction each, for a total of 20 percent.
Each month, Healthy Rewards participants will earn their premium reductions through physical activity and other activities. Participants without specific chronic conditions will be on a Preventive Track and earn premium reductions through regular physical activity, preventive screenings and online participation. Participants with certain chronic conditions will be on a Chronic Care Track which will give them access to RN and RD health coaches. Participants will track physical activity and have individualized improvement goals to earn their monthly premium reductions.
What’s New?
You spoke, and we listened! Presenting the new and improved Healthy Rewards Program.
Healthy Rewards enrollment begins now and runs through December 31, 2020*. The new program will allow you to earn a premium reduction each month of the year in 2021.
- Premium reductions will be earned MONTHLY. Those who do not achieve their goal do not have to wait a year to earn it back — it can be earned back the next month.
- Health goals are based on diagnostic measures, not preventive measures (ex: blood pressure guidelines will be <140/90 not <120/80)
- Participants can track their own progress online or through an app at any time.
- Enrollment will be through primary care providers.
- Health Coach appointments will be done primarily over the phone, by email or via Webex.
- Both spouses and colleagues covered on the Saint Francis Healthcare System medical insurance plan are eligible to participate and earn independent discounts, maximizing their reward. Other covered dependents are not eligible at this time.
- Participants will track physical activity by a device (ex: smart phone, Fitbit, Apple watch, Garmin, etc.) not self-report.
- Participants with certain chronic conditions will be on a Chronic Care Track which will give them access to RN and RD health coaches. Participants will track physical activity and have individualized improvement goals to earn their monthly premium reductions.

- Healthy Rewards runs on the calendar year. All colleagues will enroll at the same time annually.

Nothing! The worst that can happen is that you do not earn a premium reduction.

Enrollment Instructions
Enrollment is simple! Follow these steps:
- Schedule an Annual Preventative Exam with your primary care provider (MD, DO, NP, PA, APN). Be sure to schedule your appointment now. If you need to reschedule, you will still have time to complete your enrollment before December 31, 2020*.
Don’t have a primary care provider? Many providers across Saint Francis Healthcare System are accepting new patients.
Anthem provides one free “preventive exam” per calendar year. If you have had a preventive exam in the last 11 months, ask your provider to complete the Health Visit Form. Ensure all required information is provided.
Please note: If you provider diagnoses anything during your visit, it may not be considered a preventative exam, and you may pay a copay. - When scheduling your appointment, also schedule your labs — lipid panel and fasting blood sugar. If you are diabetic, also schedule an A1c test.
If you are able to have your labs drawn prior to your appointment, you will complete your Health Visit Form at your visit and you will submit the completed form to Healthy Rewards. If your labs are drawn during your appointment and results are pending, you will leave your Health Visit Form with your provider who will submit the completed form to Healthy Rewards.
Anthem offers comprehensive preventative care benefits in addition to an annual preventative exam.
To help ensure your annual wellness exam and some or all of your wellness labs are covered with no out of pocket costs to you, we are providing you with a reference list of the most common codes your provider would bill for preventative services. If the Saint Francis health insurance plan is not your primary insurance and your primary insurance has a preferred lab, please share that with your provider during your visit. - Print the Health Visit Form and bring to your appointment for your primary care provider to complete.
- Submit your completed form no later than December 31, 2020.* Submit via email or fax to Healthy Rewards at 573-331-5056. No scanner, no problem! Simply email a clear photo!
Colleagues who submitted their Health Form to Wellness should have received a confirmation email (for you, and also for your spouse, if applicable) within three business days of submission. If you did not receive confirmation, Healthy Rewards has not received your Health Form, and you should resend or follow up with your provider’s office.
*Colleagues who enroll between January 1 and 31, 2021, are still able to participate! Colleagues who enroll in January are not eligible to receive the January premium reduction but will be eligible to receive the monthly premium reduction beginning February 2021 for the remainder of the year. Colleagues who do not enroll before January 31 are not eligible to participate in the Healthy Rewards program for all of 2021 and will not be eligible to receive any premium reduction.

Frequently Asked Questions
Healthy Rewards enrollment begins now and runs through December 31, 2020. The new program will allow you to earn a premium reduction each month of the year in 2021. Please read these FAQs very carefully to ensure you get enrolled.
Who is eligible?
Full- and part-time colleagues as well as spouses who will be covered on the Saint Francis Healthcare System medical insurance plan during the 2021 calendar year.
Who is not eligible?
Full- and part-time colleagues as well as spouses who will not be covered on the Saint Francis medical insurance plan in 2021. PRN colleagues and children of colleagues are not eligible.
My spouse will be covered on my insurance but does not want to participate. Can I still earn a premium reduction?
Yes, but only half of the eligible amount. In order to earn the maximum monthly premium reduction, the colleague and spouse must both actively participate if covered on the Saint Francis medical insurance plan.
Both my spouse and I are colleagues on the Saint Francis medical insurance plan. What premium reduction will we receive?
If you and your spouse are on separate Saint Francis medical insurance plans, you can each earn a 20 percent premium reduction. If you are covered on the same plan, each of you can earn 10 percent for a total of 20 percent.
What if my spouse and/or I are covered on the Saint Francis medical insurance plan but it is secondary insurance?
The enrollment, participation and premium reduction remain the same. If the Saint Francis medical insurance plan is not your primary insurance and your primary insurance has a preferred lab, be sure to share that with your provider during your visit to ensure coverage.
I participated in Healthy Rewards in the past. Do I need to re-enroll?
Yes! Healthy Rewards is completely starting over so no one will be automatically enrolled. Anyone interested in earning a monthly premium reduction must enroll by December 31, 2020.
What is required to enroll?
All eligible colleagues and spouses must have a Health Visit Form completed by a primary care provider and returned to Wellness by December 31, 2020. If this is not completed, colleagues will not be eligible for a premium reduction during the calendar year of 2021. Colleagues are strongly encouraged to call now to schedule an appointment. If scheduled early and something happens where you cannot attend, you should have time to reschedule before the deadline. Those who complete enrollment will receive a premium reduction for the month of January 2021.
What if I received my annual preventative exam earlier this calendar year?
Ask your provider to complete the Health Visit Form with your data from your previous exam as long as the exam was completed within the last 11 months and all the required information is included. If you did not have labs drawn, ask your provider to order preventative labs and have them drawn. Once your results are in, you will need to ask your provider to complete your Health Visit Form and return it to Wellness by the deadline.
The enrollment instructions say I need to meet with a primary care provider (MD, DO, NP, PA, APN) but I do not have one. What should I do?
My OB/GYN completes my preventative exam. Is this acceptable?
Yes, as long as the Health Form is completed with the appropriate labs and by a MD, DO, NP, PA or APN, this is acceptable.
My hire month is not before the enrollment deadline. When will I meet with Healthy Rewards?
Healthy Rewards now runs on the calendar year for all participants regardless of hire month. Everyone enrolls between now and December 31, 2020.
What is different with the new Healthy Rewards program?
We listened to your feedback and created a program that allows you more flexibility in rewards and accessibility to program progress.
- Every colleague will pay the same premium rates monthly (based on the level of coverage chosen and colleague status). If you are enrolled in Healthy Rewards and meet your monthly health and wellness goals, you will be given a reduction on your premium the following
month during the third or fourth week of the month, depending on when payday falls. - In the past, if you did not meet your goals, you paid a higher premium for a whole year. Now, each month is a new opportunity to reach your goals and earn a premium reduction.
- Goals are set based on health status and chronic conditions. Everyone will have a monthly exercise goal. If you have a chronic condition, you will work with a health coach to lower your risk and manage your condition.
- Exercise will be tracked by a smart device and directly uploaded to the Healthy Rewards website. You will be able to see how you are doing by accessing the Healthy Rewards mobile app or website. Your individual goals will be listed and you can always check your progress.
Do I have to buy a FitBit or an Apple Watch to participate?
You will need a device that can track your physical activity but you can use a smart phone, Fitbit, Apple Watch, Garmin or other smart device. Healthy Rewards will provide detailed instructions on how to sync your device and upload your physical activity in the months to come. Anthem offers a
discount at Anthem.com under the “Discounts” tab.
