Your Health is Your Greatest Wealth. It is Time to Invest in You!

Healthy Rewards is a voluntary program designed to reward colleagues and spouses for taking an active role in their health. All colleagues and spouses covered on the Saint Francis Healthcare System medical insurance plan are eligible to participate.

Colleagues on the Saint Francis medical insurance plan as individuals or with children only (no spouse) can earn a 20 percent premium reduction. Colleagues with a spouse on the Saint Francis medical insurance plan can earn a 10 percent premium reduction each, for a total of 20 percent.

Each month, Healthy Rewards participants will earn their premium reductions through physical activity and other wellness tasks. Participants without specific chronic conditions will be on a Preventive Track and earn premium reductions through regular physical activity, preventive screenings and online participation. Participants with certain chronic conditions will be on a Chronic Care Track which will give them access to RN and RD health coaches. In addition, participants will track physical activity and have individualized improvement goals to earn their monthly premium reductions.

What’s New?

Healthy Rewards Program Highlights

Healthy Rewards enrollment begins now and runs through November 30, 2021*. The new program will allow you to earn a premium reduction each month of the year in 2022.

  • Premium reductions will be earned MONTHLY. Those who do not achieve their goal do not have to wait a year to earn it back — it can be earned back the next month.
  • Health goals are based on diagnostic measures, not preventive measures (ex: blood pressure guidelines will be <140/<90 not <120/80)
  • Participants can track their own progress online or through an app at any time.
  • Enrollment will be through primary care providers.
  • Health coach appointments will be done primarily over the phone, by email or via Webex.
  • Both spouses and colleagues covered on the Saint Francis Healthcare System medical insurance plan are eligible to participate and earn independent discounts, maximizing their reward. Other covered dependents are not eligible at this time.
  • Participants will track physical activity by a device (ex: smart phone, Fitbit, Apple watch, Garmin, etc.) not self-report.
  • Participants with certain chronic conditions will be on a Chronic Care Track which will give them access to RN and RD health coaches.
  • Healthy Rewards runs on the calendar year. All colleagues will enroll at the same time, September 1 – November 30, annually.

Nothing! The worst that can happen is that you do not earn a premium reduction.

Healthy Rewards Annual Enrollment Instructions for 2022

September 1- November 30, 2021

Eligibility: All full time and part time colleagues and their spouses who will be enrolled in Saint Francis Healthcare System health insurance in calendar year 2022 are eligible to enroll.

Enrollment for Current Participants

Current Participants on the Chronic Track:

If your monthly exercise goal is 100K steps/600 minutes per month, you are on a chronic care track. Call your primary care provider to schedule your annual preventive exam with your primary care provider September 1 – November 30th. Anthem covers one preventive exam every calendar year*. Once your Healthy Rewards Referral is received by Wellness, you will receive an email confirmation within 3 business days ensuring your reenrollment. Healthy Rewards Referrals must be received by Wellness no later than November 30, 2021. If you do not enroll September 1 through November 30 you will not be eligible to enroll again until January 2023.

  • If your Primary Care Practitioner is a Saint Francis Medical Partner – If your provider is part of the Saint Francis Healthcare System Medical Partners, encourage your provider to submit your Healthy Rewards Referral (formerly Health Visit Form) through EPIC during your exam. This is much easier for you and your provider.
  • If your Primary Care Practitioner is a Non Saint Francis Medical Partner – If your provider is outside of Saint Francis, you will need to print off the Healthy Rewards Referral and have your provider fax (573-331-5056) or scan and email the form to HealthyRewards@sfmc.net.

Current Participants on the Preventive Track:

If your monthly exercise goal is 150K steps/900 minutes per month, you are on a preventive track. You are not required to have an annual preventive exam your second year of participation in Healthy Rewards. However, an annual preventive exam is covered by your Saint Francis health insurance and you are encouraged to take advantage of it. You will continue to be enrolled in Healthy Rewards for 2022. Continue to check ManageWell for requirements to earn your insurance premium reduction monthly.

Enrollment for New Participants

All new participants will need to have a Healthy Rewards Referral completed at an annual preventive exam September 1-November 30, 2021.

  • If your Primary Care Practitioner is a Saint Francis Medical Partner – If your provider is part of the Saint Francis Healthcare System Medical Partners, encourage your provider to submit your Healthy Rewards Referral (formerly Health Visit Form) through EPIC during your exam.   • If your Primary Care Practitioner is a Non Saint Francis Medical Partner – If your provider is outside of Saint Francis, you will need to print off the Healthy Rewards Referral and have your provider fax (573-331-5056) or scan and email the form to HealthyRewards@sfmc.net.

If you have already had a preventive exam sometime in 2021, you can ask your provider to complete the form for you with the prior visit’s information. A lipid panel and fasting blood sugar are required. If you are diabetic, an A1C is required. These labs can be ordered with your preventive exam and are covered by Anthem. Once your Healthy Rewards Referral is received by Wellness, you will receive an email confirmation within 3 business days.

* If you had your preventive exam during the Healthy Rewards extended enrollment period – January 1-31 of 2021, SFHS will pay for you to have a second preventive exam even though it is during the same calendar year. Once you have your second exam, call Wellness at 573-331-5970 or your designated health coach, to let them know the provider and date of your second exam. Wellness will arrange for the charges to be covered. If the provider is outside SFHS, please wait and send the bill to healthyrewards@sfmc.net and Wellness will arrange for the charges to be covered.

Frequently Asked Questions

Healthy Rewards enrollment begins now and runs through December 31, 2020. The new program will allow you to earn a premium reduction each month of the year in 2022. Please read these FAQs very carefully to ensure you get enrolled.

Who is eligible?

Full- and part-time colleagues as well as spouses who will be covered on the Saint Francis Healthcare System medical insurance plan during the 2022 calendar year.

Who is not eligible?

Full- and part-time colleagues as well as spouses who will not be covered on the Saint Francis medical insurance plan in 2022. PRN colleagues and children of colleagues are not eligible.

My spouse will be covered on my insurance but does not want to participate. Can I still earn a premium reduction?

Yes. You still have an opportunity to work on your health and earn a 10% reduction separate from your spouse’s ability to earn 10%. In order to earn the maximum monthly premium reduction, the colleague and spouse must both actively participate if covered on the Saint Francis medical insurance plan.

Both my spouse and I are colleagues on the Saint Francis medical insurance plan. What premium reduction will we receive?

If you and your spouse are on separate Saint Francis medical insurance plans, you can each earn a 20 percent premium reduction. If you are covered on the same plan, each of you can earn 10 percent for a total of 20 percent.

What if my spouse and/or I are covered on the Saint Francis medical insurance plan but it is secondary insurance?

The enrollment, participation and premium reduction remain the same. If the Saint Francis medical insurance plan is not your primary insurance and your primary insurance has a preferred lab, be sure to share that with your provider during your visit to ensure coverage.

I participated in Healthy Rewards in the past. Do I need to re-enroll?

Please refer to Enrollment for Current Participants section.

What if I received my annual preventative exam earlier this calendar year?

Ask your provider to complete the Health Visit Form with your data from your previous exam as long as the exam was completed within the last 11 months and all the required information is included. If you did not have labs drawn, ask your provider to order preventative labs and have them drawn. Once your results are in, you will need to ask your provider to complete your Healthy Rewards Referral and return it to Wellness by the deadline.

The enrollment instructions say I need to meet with a primary care provider (MD, DO, NP, PA, APN) but I do not have one. What should I do?

My OB/GYN completes my preventative exam. Is this acceptable?

Yes – as long as the Healthy Rewards Referral is completed with the appropriate labs and by a MD, DO, NP, PA or APN, this is acceptable.

Do I have to buy a FitBit or an Apple Watch to participate?

You will need a device that can track your physical activity, as exercise cannot be self-reported, but you can use a smart phone, Fitbit, Apple Watch, Garmin or other smart device. Healthy Rewards will provide detailed instructions on how to sync your device and upload your physical activity in the months to come. Anthem offers a
discount at Anthem.com under the “Discounts” tab.

When are Healthy Rewards premium reductions applied?

When a Healthy Rewards participant earns a premium reduction during a month it is paid out in the second pay period the following month (not the second pay check). 

The premium reduction will be applied to these paychecks for the year 2022:

What if I have a medical condition that I am unable to achieve a goal on the track I’m on?

If you have a medical condition or your practitioner says there is a goal that is medically inadvisable for you to attempt to achieve, we will provide a reasonable alternative goal with the permission of your practitioner. Please contact Healthy Rewards at HealthyRewards@sfmc.net OR 573-331-5970 to begin the process of obtaining a medical alternative.
